Badly designed consent form leaves 50% of pupils unvaccinated

A badly designed consent form has resulted in over 50% of pupils not receiving a flu vaccine.

It appeared that parents may have missed a mandatory “consent” section of the form as it was labelled beneath a “non-consent” heading.

The form was labelled “extremely confusing” by Gosforth Central Middle School in a letter home to parents and meant that many were frustrated and concerned.

As a result, the trust recommended parents to book their children appointments to receive an immunisation.

It’s a great example of how important user-testing is for even the simplest of things such as a form with a small number of fields.”
